Commonwealth home support programme (CHSP)

Port Macquarie Occupational Therapy are a registered Commonwealth Home Support Programme (CHSP) provider and provide Occupational Therapy Services that focus on maintaining an elderly persons safety and independence in their own home.

Commonwealth Home Support Programme (CHSP) is a government funding scheme to support older adults to stay independent and active within their home and the community. This is an entry level scheme designed to support older adults to access services, including Occupational Therapy, for people 65 years and older.

Typical Services include:

Comprehensive Occupational Therapy assessment

An evaluation of your performance in daily activities, where we will identify ways to improve/maintain your independence

Assessment and referral for Home Modifications

Evaluation of your home environment with recommendations for modifications to improve your safety, accessibility and independence at home

Individualised and Group safety and falls prevention sessions

The Stepping On  programme is designed to build balance and strength and reduce risk of falls

Assessment and referral for Goods, equipment and assistive technology (GEAT)

Identification of the most suitable equipment to improve your safety, independence and engagement in daily tasks.

How to Refer to US?

CHSP referrals can come through the My Aged Care Portal. Your assessor can refer to us from within the My Aged Care Portal.

What to Expect?

Once we accept your referral, we will place you on our waiting list. Someone on our team will contact you to discuss what we can offer and complete some pre visit questions. This will allow us to get a better understanding on what services you may require.

An initial assessment with one of our Occupational Therapists will then take place in your own home with anyone that you may choose to be present. During this assessment we will discuss with you what you may be having difficulty with and how we can best support you to achieve your goals.

We will then discuss with you our recommendations to support you to achieve your goals.
