Support at Home
We provide Occupational Therapy Services funded through Support at Home. These services aim to help you live safely and independently in your own home.
OUR SERVICES INCLUDE:
- Functional Assessments
- Home Safety Assessments
- Assistive technology recommendations
- Equipment prescription and training
- Home Modifications
- Cognitive assessments and support
How to access
- An older adult is assessed by the local aged care assessment team and this can be arranged by contacting My Aged Care
- Following this assessment - You will be provided with a funding under Support at Home (Level 1-8)
- You can choose an approved Support at Home care provider of your choice to deliver and coordinate the services you need
You can access our services through Support at Home by speaking with your Care Provider.
